The nurse helps the patient into a comfortable position. Typically, for Foley catheter insertion, the patient lies on their back. If possible, knees are bent and hips slightly flexed to allow easy access.

A Foley catheter is needed when a person cannot empty their bladder naturally. Common reasons include urinary retention (the bladder doesn’t empty, e.g. due to an enlarged prostate or nerve damage), urinary incontinence (loss of bladder control causing leakage), or after certain surgeries (like prostate or gynecological surgery) where keeping the bladder empty aids healing. At home, patients who are bedridden, paralyzed, or recovering from surgery often require a Foley catheter to avoid the discomfort and danger of a full bladder. It’s also used for patients with spinal cord injuries, advanced dementia (who may be incontinent), or serious illnesses where moving to the toilet is not possible. Using the catheter at home ensures the patient’s bladder is managed continuously without multiple trips to the hospital each day to be catheterized.
Insertion can cause mild discomfort, but it should not be severely painful. Patients often describe a feeling of pressure or a brief stinging sensation. To minimize pain, our nurses use a numbing lubricant gel before insertion, which significantly dulls any pain during the catheter’s passage. We also coach the patient to take slow, deep breaths during insertion which helps relax the pelvic muscles. Most patients tolerate the procedure well, and it only lasts a few seconds until the tube is in the bladder.
Our nurse will handle all the technical aspects like insertion, removal, and scheduled changes. They will also perform or teach you the daily care routines: Hygiene: Clean the area where the catheter enters the body once a day with mild soap and water (and after any bowel movements). We’ll show you how to gently wash around the catheter without tugging it. Handwashing: Always wash your hands before and after touching the catheter or bag. This rule applies to anyone assisting the patient as well.
The most common complication is a urinary tract infection (UTI). Signs include: Fever and chills: A temperature above 38°C (100.4°F) can indicate an infection, especially if accompanied by shivering or feeling very cold (chills). Changes in urine: If your urine becomes cloudy, foul-smelling, or dark (like tea-colored or with blood tinge), this is a red flag. Normal urine should be clear to light yellow and mild-smelling. Pain or discomfort: New pain in the lower belly (bladder area), flank (sides of lower back), or a burning sensation can mean infection or blockage.
Foley catheters are designed for both short and long-term use, but they should not stay in indefinitely. Typically, a given catheter is left for up to 2 to 4 weeks, sometimes up to 12 weeks (3 months) maximum with special catheter types. Prolonged use increases infection risk and can cause other issues (mineral buildup, irritation). Our general protocol is to change the catheter every month (4 weeks) as a precaution if it’s needed continuously.
